Why does food sometimes heat unevenly in a microwave even when the timer is set correctly?

Some parts of the food become very hot while other parts remain cold. I am curious whether this is a microwave issue or something about how food is placed.

Microwaves can heat food unevenly because of how energy is distributed. Stirring, rotating and allowing food to stand briefly can improve the result.
